2015-2016 Season Influenza Vaccine Pre-Book Form
Annual flu vaccination is recommended for ALL, age 6 months and up

Tennessee’s VFC program needs all VFC providers to pre-book your VFC flu vaccine orders ONLINE at the Tennessee Web Immunization System (TWIS) website: http://twis.tn.gov.

Pre-bookedorders will be shipped automatically in partial shipments as soon as we begin receiving doses, probably in August. Timing of shipments is unpredictable. The additional steps for VFC vaccine distribution adds > 2 weeks to distribution schedules, relative to private sector flu vaccine released at the same time. Please request vaccine based upon your reasonable expectation of doses that will be used.

Click the link labeled Pre-book Flu in the center of the main public page of our Immunization Registry (TWIS) website: http://twis.tn.gov. You do not have to log in or use VIM to pre-book. Save a copy of your pre-book for your records!

Attention: VFC providers who do not have internet access need to contact the VFC Program at (615) 253-6915 to request a Pre-book Flu form to fax – the submission deadline is the same.

VFC providers who do not pre-book influenza vaccine: We will pre-book a bulk quantity of vaccine to provide some vaccine to practices that do not pre-book. However, these practices will receive later shipments of vaccines and may not have the quantities or formulations they desire at that time. If we run out of pre-booked vaccine, CDC typically does not allow us to order more until late season (December).

When calculating quantity: All people age 6 months and older need annual influenza vaccination. Factor in the need for 2 doses for children 6 months through 8 years receiving flu vaccine for the first time, as well as those in that age group who have received only 1 dose since July 2010. All others need 1 dose. Consider your vaccine storage space: Pre-filled syringes (PFS) will take up 5-10 times the storage space of the same number of doses in multidose vials (MDVs).

Vaccines available: All FluMist nasal spray will be quadrivalent (containing 2 type A strains and 2 type B strains). Most injectable vaccine will be quadrivalent ( containing 2 type A and 2 type B strain); but some trivalent product will also be available.
